- If you do not have a passport, you may instead bring both your birth certificate and a government-issued I.D.
- If the surname on the birth certificate does not match the surname on your government-issued I.D., you must bring a connecting document (example: marriage certificate)
- Please list name BY ROOM – not by household – if you are sharing a room with someone from another household please list them with the person they are sharing a room with.
