Please submit all fields on this form to be considered to receive 50% of the cost of the event reimbursed through your preferred reimbursement partner.

You DO NOT have to commit to this event 100%, we just have to set aside funds NOW even if it is only a potential event. When the time gets closer, you can let us know if you want to go ahead with the event.

Deadline date to pay

This will be the date given by the event organizer or publication.

Event or campaign start date

If it’s a one day event, start date will be the same as end date. If it’s a publication, the start date will be the beginning of the publication’s run.

If it’s a one day event, end date will be the same as start date. If it’s a print ad, the end date will be the end of the publication’s run.

Start and end time of event

This should be the hours the event is open, eg. 8am-4pm, 12pm-6pm, etc. IF YOU ARE DOING A PRINT AD YOU CAN PUT “N/A”

Event setup Time

The time that you are asked to be there to begin setup for the event. If the setup date is different than the “Start Date” given above, please include the setup date. IF YOU ARE DOING A PRINT AD YOU CAN PUT “N/A”

Name of Venue of Publisher

Please list the name of the venue including the address with street number, street name, city, state, and zip code and anything specific, like “Grand Ballroom” or “Event Room” or other specifics related to the location of the event. IF YOU ARE DOING A PRINT AD, PLEASE LIST THE PLACE THIS WILL BE PUBLISHED OR PRINTED. eg, Grove High School, Royse City, Dallas Junior League, etc.

Provide a contact name, phone number, and email for the event coordinator who represents the company providing the event or publishing the print media

What is the estimated attendance of the event, or audience for your print media?

The event or publication can usually provide this range of attendance. If you’re hosting the event include how many you have invited.

TOTAL cost to participate in the event

How much is being paid to the event company or publisher in total? This should be the total amount you paid or are required to pay to participate in this event, or publish your print ad.

Partner co-op amount: the amount to be reimbursed by your chosen co-op partner, eg. WDTC/Disney, Sandals, etc.

This should be 50% of the total cost listed in the question above to participate in the event or publication.
