I hereby authorize Happily Ever After Travel to use the information provided to process payments for the travel arrangements associated with this reservation and to submit such payments directly to the applicable travel suppliers. I certify that I am the authorized cardholder or otherwise have authority to use this credit card for the transaction described herein.

I acknowledge that I have reviewed my reservation details and confirm that all traveler information, itinerary details, and payment information are accurate. I understand that changes to my reservation may result in additional charges, supplier-imposed fees, or penalties.

I understand that Happily Ever After Travel acts solely as a travel advisor and intermediary on my behalf and does not own, operate, or control the travel services provided by airlines, cruise lines, hotels, resorts, tour operators, transportation providers, insurance companies, or other travel suppliers. Any complaints regarding services received during travel must be addressed with the applicable supplier. Dissatisfaction with services provided by a travel supplier does not automatically entitle me to a refund or reversal of authorized charges.

I acknowledge that I have reviewed and accept the cancellation, refund, change, and payment policies applicable to my reservation. I understand that many travel arrangements are non-refundable and that cancellation penalties may apply.

I agree not to initiate a chargeback or payment dispute for any charge that I have authorized and that corresponds to the travel arrangements purchased. Should a concern arise regarding my reservation, I agree to contact Happily Ever After Travel and/or the applicable travel supplier to seek resolution prior to disputing the charge with my financial institution.

I understand that I am solely responsible for obtaining valid identification and carrying valid passports, visas, travel documents, health documentation, proof of citizenship, and any other documentation required for travel. I acknowledge that failure to obtain or present required documentation may result in denied boarding, denied entry, cancellation of travel services, or additional expenses for which I am solely responsible.

I understand that I am responsible for complying with airline, cruise line, and supplier check-in requirements and recommended arrival times. Failure to meet such requirements may result in denied boarding or forfeiture of travel services without refund.

I acknowledge that I am responsible for meeting all payment deadlines associated with my reservation. Failure to make payments by the supplier's due dates may result in cancellation of my reservation, forfeiture of deposits, additional fees, or loss of promotional pricing.

I further agree that my electronic signature and submission of this form shall have the same legal effect as an original handwritten signature and shall constitute my acceptance of all terms contained herein.

I consent to the collection, use, and transmission of my personal information as necessary to facilitate the booking and fulfillment of my travel arrangements. I understand that certain personal information may be provided to travel suppliers, governmental authorities, and other third parties as required for transportation, accommodations, security, immigration, or other travel-related purposes. Happily Ever After Travel is not responsible for the privacy practices, policies, or actions of such third parties once information has been transmitted as necessary to complete my travel arrangements.
