I understand that coverage is subject to the terms, conditions, limitations, exclusions, and eligibility requirements of the travel protection provider and policy selected. I understand that coverage is not effective until purchased and confirmed.

I acknowledge that travel protection has been offered to me and that I have received pricing and information regarding the available travel protection plan(s).

After considering my options, I voluntarily decline travel protection for this reservation.

I understand that by declining travel protection, I assume the financial risks associated with unforeseen events that may impact my travel plans, including, but not limited to:

• Cancellation penalties
• Trip interruption expenses
• Medical expenses incurred while traveling
• Emergency medical transportation or evacuation
• Travel delays
• Missed connections
• Lost, stolen, or delayed baggage
• Supplier cancellation policies and non-refundable deposits
• Weather events
• Other unforeseen circumstances that may result in the loss of prepaid travel expenses

I understand that many travel arrangements are non-refundable and that cancellation penalties may apply immediately or increase as the travel date approaches.

I further understand that if I choose to purchase travel protection at a later date, coverage may no longer be available, certain time-sensitive benefits may no longer apply, or the premium may change.

By signing below, I certify that:

• I am the primary traveler or an individual authorized to make travel planning decisions on behalf of every traveler included on this reservation.
• My election above applies to all travelers included in this reservation unless otherwise requested in writing.
• I have had the opportunity to ask questions regarding available travel protection options before making this decision.
• I acknowledge that Happily Ever After Travel has offered travel protection and provided pricing for the available plan(s).
• I understand that Happily Ever After Travel and its travel advisors are not insurance companies or insurance adjusters and cannot interpret, modify, or guarantee insurance benefits, exclusions, or claims decisions. Questions regarding coverage should be directed to the travel protection provider.
• I understand that if travel protection has been declined, Happily Ever After Travel cannot be held responsible for financial losses that may have been covered under an available travel protection plan.
